Hi Eric, the artist is Paul Wenzel and the medium is acrylic paint on illustration board.Freaky, great piece, isn't it!
I love this! It's like "Hayley Mills as Sybil!"It also is an ancestor to the "Freaky Friday" art featuring the welded heads of Barbara Harris and Jodie Foster.
